With increasing cases of COVID-19 and the recent orders from the Public Health Officer, it is now essential that businesses have daily screening measures in place for their employees. We wanted to share how you can use Forms in Office 365 for a quick and easy way to allow your staff to submit the health check while compiling the results into a spreadsheet.
How to Create a Form in Microsoft Forms
- Go to https://forms.office.com and sign into your Office 365 account.
- Select New Form.
- Enter the desired name and an optional subtitle for the form.
- Click Add Question to add a new question. Here are some of the question types you can choose from: Choice, Text, Rating, Date, Ranking, and more.

- Some question types such as Choice may require you to enter additional details such as the text you want to display for each of the choices.
- Click Add Question for each additional question. You can change the order of questions by clicking the Up and Down arrows located to the right side.
- Set further restrictions, if necessary, by clicking more settings for question x.
- Click Preview to see a final copy of the form. Here you can test the form by entering answering to the questions and then clicking Submit.
- Click Mobile to see how your form will look on mobile. You can exit this view by clicking Back.
- Start collecting responses by clicking Share. Here you can copy a link to share with your employees. You can also download and print a QR code that your visitors can scan to access the form.
NOTE: Alternately, if you wish to have a persistent spreadsheet, you can start by first creating a new spreadsheet online within Office 365, then proceed to the Insert tab > Forms.
Sample COVID-19 Health Check Questions
*These can all be setup as Choice questions with answer options Yes and No. We suggest setting all COVID-19 screening questions as required to ensure your employees and visitors don’t miss any questions.
- Are you displaying any of the following new or worsening symptoms? – fever or chills, couch, loss of sense of smell or taste, difficulty breathing, sore throat, loss of appetite, extreme fatigue or tiredness, headache, body aches, nausea or vomiting, diarrhea.
- Have you travelled outside of Canada within the last 14 days?
- Have you been identified by Public Health as a close contact of someone with COVID-19?
- Have you been told to isolate by Public Health?
How to Review Form Submissions
- Go to https://forms.office.com and sign into your Office 365 account.
- Select your COVID-19 Health Check Form.
- Click Responses at the top of your form.
- Click View Results to view all submissions.
- Alternatively, click Open in Excel to download a copy of all of the data in a spreadsheet.
